Am I correct in assuming that in 1966 the brake and clutch lever perches had holes for mirrors but the actual mirror was an option?

Yes, ....and no. Dealers were asked to order machines with at least on trim group (in early years they were required to order a trim group, even the most basic), and every Sportster trim group included a mirror. So base models didn't have one as built, but virtually no base models were sold through dealers.
